Report CDS/Marijuana Loss

Pursuant to Title 63 Section 2-309(A)12 of the Oklahoma State Statutes, as well as Title 475 Chapter 20-1-8, Registrants are required to notify OBNDD of any loss or significant theft of registration certificates, order forms in addition to any diversion or suspected diversion of controlled substances in a manner approved by the OBNDD. 

• Acceptable notification shall be submitted on the DEA106 Form for non-medical marijuana registrants.

• Acceptable notification by medical marijuana registrants shall be in a manner that is clear and concise that identifies the quantity, estimated value and item identification of the product or products.
